Introduction Welcome to Lighthouse Therapy! These Terms of Service ("Terms") govern your use of our teletherapy services. By accessing or using our services, you agree to comply with these Terms. If you do not agree, you may not use our services.
Services Provided Lighthouse Therapy provides teletherapy services, including but not limited to:
Speech-language pathology, occupational therapy, physical therapy, and behavioral and mental health therapy. These services are provided by an appropriately licensed and/or certified clinician per appropriate state rules/guidelines.
Direct therapy sessions, makeup/compensatory services, assessments, and consultations with parents or teachers.
Case management and coordination of services.
Coordination and provision of indirect therapy time, including documentation and reporting.
Certified Special Education services are provided by an appropriately credentialed teacher per applicable state rules/guidelines.
Case management and coordination of services.
Special education instruction as defined by applicable state rules /guidelines.
Access to an online Individualized Education Plan (IEP) system.
Additional services as agreed upon in a written addendum.
User Responsibilities By using our services, you agree to:
Provide accurate and complete information as required for service enrollment.
Comply with all applicable laws, regulations, and policies.
Ensure that students attend scheduled therapy sessions.
Maintain the confidentiality of any provided login credentials and personal data.
Payment and Billing
Fees for services are outlined in an applicable rate sheet.
Payment is due within 15 days of invoice receipt.
A discount may be applied for early ACH payments.
Late payments will incur a 2% monthly interest fee.
Any billing disputes must be reported within five (5) days of invoice receipt.
An initial payment may be required prior to the start of services.
Clients will be invoiced monthly based on service utilization.
Confidentiality & Privacy Lighthouse Therapy adheres to all applicable privacy laws, including the Family Educational Rights and Privacy Act (FERPA) and the Health Insurance Portability and Accountability Act (HIPAA). We will not disclose any confidential information without proper authorization unless required by law.
Intellectual Property
All materials, reports, and resources provided by Lighthouse Therapy remain our property.
Users are granted a non-transferable license to use provided materials solely for educational purposes.
Users may not reproduce, distribute, or sell any materials without written permission.
Termination & Cancellation
Either party may terminate services within 30 days of the initial signing of the agreement. Written notice of termination is required.
After the initial 30 days, if a client wishes to terminate services or to reduce services and/or student count by more than 10% of the agreed upon threshold, a 90-day notice will be required.
Lighthouse Therapy reserves the right to suspend or terminate services if a user violates these Terms or fails to make payments.
No refunds will be issued for partially completed service periods unless otherwise stated in a written agreement.
Liability & Indemnification
Lighthouse Therapy is not responsible for indirect, incidental, or consequential damages resulting from service use.
Users agree to indemnify and hold Lighthouse Therapy harmless against any claims arising from misuse of services.
Insurance reimbursements for required coverage must be processed within 15 days of request.
Governing Law These Terms shall be governed by the laws of the State of Texas. Any legal disputes will be resolved in the courts located in Corpus Christi, Texas.
Amendments We reserve the right to modify these Terms at any time. Any updates will be posted on our website, and continued use of our services constitutes acceptance of the revised Terms.
